Making a Referral for a Heart Transplant

We offer effective long-term treatments for advanced heart failure.

If a patient has been diagnosed with heart failure, it’s important to be referred for a comprehensive evaluation of the condition as soon as possible. A primary care physician, cardiologist, or cardiac surgeon can refer patients to the Miami Transplant Institute for a heart failure and transplant evaluation. Patients can also contact our team directly to schedule an appointment.

Our heart transplant team can draw on the comprehensive resources of Jackson Health System to provide multidisciplinary care—an important consideration because patients’ medical condition may affect other organs as well as the heart.

Most patients with heart failure have other medical problems and can benefit from a comprehensive evaluation. Our multidisciplinary transplant team receives referrals from patients who have been recently diagnosed as well as those who have been admitted to hospital intensive care units (ICUs). We have the ability to address even the most challenging cases.
